If you operate a limousine service, you will fill out the luxury limousine permit application.

The actual permit application may be a little different from the one shown in this help page, but you should be able to get the general idea.

Page	Description
Instructions	The instructions page gives you an idea as to how much the permit costs as well as how much and what type of insurance you will need in order to comply with statute. Insurance levels depend upon the seating capacity you intend to provide service for. Your insurance company must fill out and submit the indicated forms through our on-line motor carrier insurance system.
Business	The business page allows you to provide information about your company. This page is described in more detail here.
Owners	You must enter the names and titles of the owners of your company.
Agent	Specify a business or individual that is authorized to fill out this form and operate on your behalf. For more information, go here.
Numbers	Use the numbers page to enter any special identification that you have obtained on behalf of your business. The Colorado PUC uses these numbers to verify your identity and to ensure that you are paying the correct application fee (under some circomstances, the application fee can be waived for applicants that have already paid federal application fees.
