The Hazmat Annual Permit Application
If you transport hazerdous materials in the State of Colorad, you will
fill out the Hazmat Annual permit application.
The actual permit application may be a little different from the one
shown in this help page, but you should be able to get the general idea.

The instructions page gives you an idea as to how much
the permit costs as well as how much and what type of insurance
you will need in order to comply with statute. Fees are based on
the number of vehicals you use to transport material. Your
insurance company will fill out and submit the indicated forms
through our on-line motor carrier insurance system.

The business page allows you to provide information about your company.

| Owners |
You must enter the names and titles of the owners of your company.

| Agent |
Specify a business or individual that is authorized to
fill out this form and operate on your behalf. For more
information, go here.

| Numbers |
Use the numbers page to enter any special identification that
you have obtained on behalf of your business. The Colorado
PUC uses these numbers to verifyl your identity and to ensure
that you are .

| Carrier |
Choose the options that best describe the type of business
you operate and click the Continue button.

| Safety |
Enter the date of your most recent FMCSA compliance review.
Provide the rating your received during your review. Press the
Continue button.

| Requirements |
The page lists all of the different types of hazerdous materials
that you could transport. Check on all the options that apply to
you. Answer the Yes/No questions at the bottom of the page and
press the Continue button.

Check the box that best corresponds to the size of your operation
and press the Submit and Pay button.
