The agent page is used in two places:

1. The second page of the HOA Registration wizard
2. The second tab of the Modify Home Owners Association... page.

Use the agent page (see figure 1) to tell the Division of Real Estate who is responsible for the registration of the home owners association.

You must choose one of the two options listed at the top of the page:

• I am the designated agent.
• Someone else is the designated agent. Note: choose this option only if the representative is not a currently registered user of the HOA Registration system.

If you chose the latter, fill in the remainder of the form. Each field therein is described in more detail in what follows.

Agent Name	The full, legal name of the person responsible for reporting the home owners association.
Management Company Name	The name of the company that employs the named agent.
Number of Units	The number of units currently under management of the home owners association.
Phone Number	The main phone number of the home owners association.
Fax Number	The fax number of the home owners association.
Physical Address	Enter the physical address of the HOA's representative.
Mailing Address	If different from the physical address, supply the mailing address of the HOA's representative. If the mailing address and the physical are the same, check the box labelled Use Physical Address.
