Use the contact page to supply your contact information (see figure 1).

Supply a value for each field and press the Continue button when you are satisfied that the information you entered is correct. Each section is described in more detail in what follows.

Name	The name of the person requesting access to the HOA Registration System
Email Address	Your email address.
Phone Number	The telephone number that you use most frequently and by which the Division of Real Estate will be able to contact you.
Fax Number	Optional, the phone number where you would like to receive faxes.
Mailing Address
Street Address 1	Street address of the person requesting access to the HOA Registration system.
Street Address 2	Apartment or Unit number.
Zip	Five digit postal code.
City	City of the person requesting access to the HOA Registration system.
State	Select the state from the drop down box.

The navigation buttons appear on each page of the wizard and allow you to move from one page to the next and back again. Each button is described in more detail:

Back	The back button navigates to the Account page.
Continue	Navigates to the Role page of the wizard.
Cancel	Returns to the HOA Registration homepage without saving any information.
