The association page is used in two places:
- The first page of the HOA Registration wizard
- The first tab of the Modify Home Owners Association... page.
Use the association page (see figure 1) to describe a home owners association.

Each field is described in more detail in what follows.
| Name |
The full, legal name of the home owners association. |
| Secretary of State ID |
The ID number of the home owners association as assigned by the Colorado Secretary of State's office. |
| Number of Units |
The number of units currently under management of the home owners association. |
| HOA Type |
Choose one of the following options:
- Condominiums - A community in which portions of property are designated for separate ownership and the remainder is designated for undivided common ownership.
- Cooperatives - Community in which the real property is owned by the association and each member owns a part of the association and is therefore entitled to possessory interest in their unit.
- Planned Communities - A community that is not a cooperative or condominium.

| Street Address (two fields) |
The street address of the home owners association. If the HOA has a street address and a post office box, you may enter both.

| Zip |
The postal code of the home owners association. The HOA Registrations
website makes use of smart zip codes. If the system knows the zip code
you enter, the city and state fields are automatically filled in with
the corresponding values. |
| City/State |
Supply the city and the state where the association is headquartered. |
| Phone Number |
The main phone number of the home owners association. |
| Fax Number |
The fax number of the home owners association. |
| Annual Revenue |
Choose the option which best describes the HOA:
- HOA has annual revenues of more than $5,000.00.
- HOA has annual revenues of less than $5,000.00.
- HOA has no revenue and is not authorized to make assessments.
