Dentist, Dental Hygienist and Academic Dentist Renewal 2012

Renewal Postcards:

This renewal period you will receive a yellow RENEWAL NOTICE POSTCARD.  There is no personal information on this postcard (other than your name, address and license number).  This is the only notice you will receive.  Please let other people in your household know that this yellow postcard is not junk mail! Click here to see a sample. If you have a current email address on record, you will also receive notification about renewing your license. These are the only notices you will receive regarding your renewal. 

Important Notice: Regarding Dental Renewal Postcards: We are aware that some renewal notice postcards were mailed to some licensees with the incorrect License Type or License Status listed. This was a printing error and your records in the system are accurate. You will be able to renew your license as normal in the correct type and status as you will see once you log into your account. We apologize for any confusion this may have caused.

Instructions for Online Renewal:

Dentist, Dental Hygienist and Academic Dentist licenses expire on February 29, 2012 and it is now time to renew your license to keep it in a current status until February 28, 2014. To renew your license, please follow the below steps:

Note: Before moving forward you must complete your Healthcare Professions Profile requirement. This requirement is new to the Dental renewal for 2012 and is required of all Dentist, Dental Hygienist and Academic Dentist licenses in Active and Retired status. Please read the below section titled Additional Renewal Requirements to determine the required items for your particular license type. If you have not completed your Profile, please click here to continue to the Healthcare Professions Profiling prior to accessing the Online Renewal System.

  1. After reading all information listed here and ensuring you have completed and submitted your Healthcare Professions Profile accessible from the link listed above, click on the link below titled "Click here to go directly to online renewal".
  2. Click "I have an individual license" then click on the "Continue" button.
  3. If you are a returning Registrations Online Services user, enter your SSN and password in the fields on the right. If you do not remember your password or if you are a new user and do not have a password, click the link labeled "Set up password" on the left, then follow the instructions to set up your password. NOTE: The passwords for the renewal site will be different than the password for the Healthcare Professions Profile site. Renewal site passwords must contain letters and/or numbers and cannot have any special characters (!@#$%^&*).
  4. Once you are logged into your account you will be able to update your contact information. To do so, click on the button labeled, "Update Contact Information" then complete the necessary fields to update your information, e.g. mailing address, phone number, email address.
  5. When you are ready to renew, click the button labeled "Renew this License" and follow the instructions.
  6. Complete the Affidavit of Eligibility and Renewal Attestation (if present) requirements and click submit. After reviewing the fees on the following page, you may click the "Pay Now" button.
  7. You will pay for your renewal using our secure payment system. You must pay by Credit Card (Visa, Master Card, Discover, American Express), by Debit Card (as long as it bears the Visa or Master Card logo) or by Electronic Check (eCheck) by using your bank routing number and checking account number.
  8. After entering your credit card/eCheck information and clicking continue you will enter the appropriate billing information. Make sure to complete all required fields and click continue. Note: The required fields are designated by a blue line.
  9. You will then be able to review/verify your payment information on the following screen. If you need to make any changes/updates you may click the "edit" button in the respective module. If all information is correct, you may click the "Agree/Continue" button at the bottom of the page to complete the transaction.
  10. Once complete you will see a confirmation page. Make sure to print this page for your records. You will also have a copy of this confirmation emailed to you if you entered your email address on the billing information page (you must make sure that the email address, support@www.colorado.gov is added to your approved senders list to receive email confirmation). This will complete your transaction. Your license expiration date will be updated the following business day and you will receive your renewed wallet license card within 7-10 days.

License Renewal Fees:

  • If renewed on or before February 29, 2012 :
    • The Active Dentist renewal fee is: $412
    • The Inactive Dentist renewal fee is: $394
    • The Retired Dentist renewal fee is: $20
    • The Active Dental Hygienist renewal fee is: $86
    • The Retired Dental Hygienist renewal fee is: $20
    • The Active Academic Dentist renewal fee is: $380
  • If renewed on or after March 1, 2012 , a $15 delinquency charge is applied and the totals are:
    • The Active Dentist renewal fee is: $427
    • The Inactive Dentist renewal fee is: $409
    • The Retired Dentist renewal fee is: $35
    • The Active Dental Hygienist renewal fee is: $101
    • The Retired Dental Hygienist renewal fee is: $35
    • The Active Academic Dentist renewal fee is: $395
  • After April 30, 2012, your license will be lapsed and you must reinstate.

All fees are Non-Refundable. The Division of Registrations cannot and will not take payment information over the phone. We will not request credit card numbers or checking account numbers via telephone. You will only be able to pay for your renewal online, in person at our office or by mail with the necessary renewal packet. To protect your information DO NOT provide any credit card or checking account numbers over the phone to an unknown source.

Additional Renewal Requirements:

Healthcare Professions Profile: Because of the success of the Physician Profile program, Senate Bill 10-124, the Michael Skolnik Medical Transparency Act of 2010 (the Act), was passed expanding profiling to include more than 18 other professions. Because of this expansion, it is now referred to as the Healthcare Professions Profiling Program (HPPP) and the administration of the program is no longer under the Colorado Medical Board and is now under the Division of Registrations. The Act requires all Dentists, Dental Hygienists and Academic Dentists who submit an application for an initial license, license reinstatement, reactivation, or renewal in Active or Retired status, to disclose specific information that can be accessed by the public. BY LAW, a Dentist, Dental Hygienist or Academic Dentist license CANNOT be renewed in Active or Retired status unless the licensee complies with the profile requirements.

Renewal Attestation (Dentists and Dental Hygienists Only): You must complete the mandatory 2012 Dentist/Dental Hygienist Renewal Attestation prior to renewing your license. You will be prompted to do so through the Online Renewal System. Once complete, you will be able to move forward and pay for your renewal.

Military Exemption HB11-1013:

Disclosure of Criminal Conduct:

During the licensing process, the Division asked whether you have ever been disciplined in any state, arrested, charged, convicted or pled guilty to a crime. After initial licensure, all licensees have a duty to report subsequent felony convictions and disciplinary actions. Some must also report misdemeanors related to the specific practice. Failure to disclose could constitute grounds alone for revocation of your license.

Licensure is a privilege, not a right. In order to obtain and retain the privilege, you must be completely honest. The Division conducts annual audits of its licensing database against several criminal and national disciplinary databases, which allows the Division to track subsequent criminal conduct and disciplinary actions after initial licensure.

An arrest, disciplinary action or subsequent criminal conviction does not mean an automatic revocation of your license. The board or program will look at the facts surrounding the criminal conduct and disciplinary action to determine whether you are still fit for licensure. Your license will not necessarily be revoked if you have been disciplined, arrested, charged or convicted, but your license will most likely be revoked if you fail to disclose it.
