Address Change Request

FOR DIVISION OF REGISTRATIONS LICENSEES ONLY:
Change your name || Request a duplicate license

Most licensees are able to complete routine changes to address or contact information by using Registrations Online Services. There are some exceptions:

  • Licensees without a U.S. Social Security Number
  • Business licenses or registrations
  • Licensees with limited computer access

If you are unable to change your address online, you must:

Download and print the Address / Name Change / Duplicate License Request Form. *

* If you have a fax machine, this form can be faxed directly to you via our FaxBack program.
Simply call (303) 894-7800, select option #2, then option #5, and follow the instructions for the FaxBack program.

Fill out the form completely.

Attach any required documentation or fees.

Name change requests require a copy of the legal documents documenting your name change. Acceptable forms of documentation include: marriage license, divorce decree, court order, immigration records, or passport.

Your name change cannot be processed without this supporting documentation!

Submit the completed form and any required documentation or fees to the Division.

Disclosure of Addresses: Some licensees have expressed concern that their residential address is accessible. Consistent with Colorado law, all addresses on record with the Division are public record and must be provided to the public when requested. In most cases, it is permissible for an individual to provide an address of record other than a residence, such as a post office box or a practice location. Please be advised that all correspondence, including renewal notices, licenses, and other legal documents, will be mailed to the address provided.

Renewal notices are mailed to the last address of record approximately six weeks prior to the expiration date. We recommend you check your address and be sure it is accurate at least two months before your license expires. Address changes may be made at any time using Registrations Online Services.
