1	APPLICATION FOR CHANGE IN STATUS REC-BL (07-08) a. Activate or inactivate a license b. Transfer license to a different company c. Upgrade level of authority d. Change a personal name e. Become licensed as an individual proprietor with or without a trade name, or to change the name and/or the address of acurrently licensed individual proprietorship f. Change the name, address or broker of a currently licensed corporation, partnership, or LLC g Change the resident address of the licensee h. Reinstate or renew a license include the AFFIDAVIT OF ELIGIBILITY
2	APPLICATION FOR COLORADO REAL ESTATE BROKER LICENSE BA 2-09 Someone using this application has met the requirements for licensure under the 1997 revised license law.
3	ADDENDUM TO APPLICATION FOR COLORADO REAL ESTATE BROKER LICENSE BAA This form is required if you have answered "yes" to any of the questions in item #12 of the APPLICATION FOR A COLORADO REAL ESTATE BROKER LICENSE .
4	APPLICATION FOR A NEW CORPORATION, PARTNERSHIP OR LLC REAL ESTATE LICENSE CPA This form is to apply for a Corporation, Partnership, or Limited Liability Company License.
5	APPLICATION FOR LICENSE AS TEMPORARY CORPORATE, PARTNERSHIP, OR LIMITED LIABILITY CO. REAL ESTATE BROKER REC-TEMP This form is to apply for a Temporary Corporation, Partnership or Limited Liability Co. as a Real Estate Broker.
6	REQUEST FOR A PRELIMINARY ADVISORY OPINION REGARDING APPLICATION FOR A COLORADO REAL ESTATE BROKER LICENSE. PAO preliminary advisory opinion pursuant to Rule A-12(b)(1) and (2) regarding the potential effect that previous conduct, criminal convictions or violations of the real estate license law may have on a formal application for licensure as a Colorado Real Estate Broker.
