Colorado High Cost Support Mechanism Worksheet and Declaration of De Minimis Form

Who must file?

• All Telecommunications carriers providing intrastate telecommunications services within the state of Colorado must file a Colorado High Cost Fund Worksheet /or Declaration of De Minimis Form. 

• Contributors that provide intrastate telecommunications, but whose contribution would be de minimis, are not required to contribute to the CHCSM. A contributor is considered de minimis if its contribution for the calendar year is less than $10,000. However, all Incumbent Local Exchange Carriers (ILECs) and Eligible Telecommunications Carriers (ETCs) must file the Worksheet and contribute to the CHCSM.
