Telephonic Reporting
NRC Reportable Incidents (Part 191.5)
Operators of pipelines are to make telephonic notice to the Public Utilities Commission, Pipeline Safety Section (303-894-2854) when a reportable gas event or incident occurs.
A Telephonic report to the PUC does not relieve the operator from making the telephonic notice to the Federal National Response Center (NRC) as required under part 191.5.
A Telephonic Report is required for any incident that leads to a release of gas and:
- an injury necessitating in-patient hospitalization, or fatality; or
- an estimated property damage equal to or greater than $50,000 (excluding cost of gas loss); or
- unintentional estimated gas loss equal to or greater than 3000 mcf .
Guide to Rescind Telephonic Incident Reports. If, after the operator's initial investigation, it is determined that the event did not meet the PUC or the PHMSA reporting requirements, the operator can rescind the initial telephonic report. Please include the assigned NRC number on all correspondence to the address below.

Additional Telephonic Reporting Requirement (Colorado PUC only, Rule 4911 - Distribution /Transmission /Gathering - (Class 1 ,2, and 3 areas))
The following events are required to be telephonically reported to the PUC at 303-894-2854:
- Gas leak resulting in the evacuation > 50 people from a normally occupied building.
- Gas leak which results in a road or railroad closure.
- Immediate Repair (as defined in Rule 4901(k)).
Reporting of NRC Incidents and Safety Related Conditions (SRC)
NRC Incident: Distribution /Transmission /Gathering (Class 2 & 3 areas)
For all pipeline incidents/accidents occurring on or after January 1, 2010, PHMSA requires operators to file the applicable newly approved pipeline incident and accident reporting forms for Gas Distribution Systems, Gas Transmission & Gathering Systems, and Hazardous Liquid Pipeline Systems at the new Electronic Incident Accident (EIA) forms system website at the following URL http://pipelineonlinereporting.phmsa.dot.gov/. Your operator ID and PIN you currently use for the ODES system will be the same for the new EIA system.
If for any reason you're unable to submit your report online, please send your hard copy to the attention of the Information Resources Manager at the address listed in the instructions. Any questions regarding this requirement can be directed to the PHMSA operator helpline at 202.366.8075.
