Utility Designee form - the appropriate form must be completed by all telecommunications utilities upon CPCN approval and be updated as utility information changes. The Commission has opened a repository docket, Docket No. 12M-Designee, to facilitate parties E-Filing their Utlity Designee form. This docket number is to be used in 2012 in conjuction with an initial or update Utility Designee filing. |
Colorado Department of Revenue Form DR 525 has been incorporated into the both annual reporting workbooks:
"Annual Report of Public Utility Intrastate Gross Operations Revenue" - this form DR 525 must be filed in addition to the Annual Report required by this Commission for maintaining your certification or registration and is due to the Commission by April 30th of each year for the prior calendar year's gross Colorado Regulated Operating Revenues. Annual Reports are due April 30th of each year for the previous calendar year. This DR 525 report MUST also be filed with the Colorado Department of Revenue on or before May 2, 2011. |
DeregulationEffective October 19, 2005, each utility holding a Certificate of Public Convenience and Necessity or Letter of Registration granting it authority to provide intraLATA toll services, in accordance with Title 40 of the Colorado Revised Statutes, may file an Application for Deregulation of IntraLATA Interexchange Telecommunications Services using this prescribed form: All telecommunications utilities must continue to submit any and all reports required of telecommunications providers, including the annual financial report, for the period it offered this telecommunications service in Colorado from January 1, 2005 through the effective date of Commission's Decision granting the authority to deregulate its intraLATA toll services, and must remit any assessments and fees due and owing the Commission. |
