525 form (Colorado Department of Revenue Annual Report) is due May 2nd this
year for the prior calendar year's gross Colorado Regulated Operating Revenues. Entities may not request an extension of time to file this form.

On or before April 30, 2011, each utility holding a Certificate of Public Convenience and Necessity to provide electric, gas, or steam service in Colorado during the 2010 calendar year shall file an annual report in the form prescribed in this Order, and any additional reports as required by Appendix A to this Order.
Utilities shall also submit a copy of the Colorado Department of Revenue Form DR 0525 no later than April 30, 2011. Entities may not request an extension of time to file this form.

This form,
DR 0525, must be filed in addition to the Annual Report required by
this Commission for maintaining your certification or registration and is due
to the Commission by April 30th of each year for the prior calendar year's gross Colorado Regulated
Operating Revenues.
This DR 0525 report MUST also be filed with the Colorado Department of Revenue on or before
May 2, 2011.
