Filing Requirements

Maintaining Your Certificate as a Provider of Telecommunications Services


Commission Rule 4 Code of Colorado Regulations (CCR) 723-2, outlines requirements: for a certificate of public convenience and necessity to provide local exchange telecommunications services; for a letter of registration to provide emerging competitive telecommunications services; to discontinue any telecommunications service; to execute a merger or transfer; and registration as a toll reseller. These rules are issued pursuant to §§ 24-4-103, 40-2-108, 40-15-301(2), 40-15-302(2), 40-15-302.5, 40-15-305(2), and 40-15-503(2), C.R.S.

Tariff Filings

Telecommunications carriers, with the exception of Toll Resellers, are required to file and maintain tariffs with the Commission.
Utility Designee Form

Utility Designee form - the appropriate form must be completed by all telecommunications utilities upon application approval and be updated as utility information changes.

The Commission has opened a repository docket, Docket No. 12M-Designee, to facilitate parties E-Filing their Utlity Designee form. This docket number is to be used in 2009 in conjuction with an initial or update Utility Designee filing.

Annual Report Requirements

Telecommunications carriers in the following categories are required to file annual reports by April 30th of each year for the prior calendar years activity:

Report of Revenues

Telecommunications carriers are required to report revenues annually to the Department of Revenue by using the DR0525 form included in the Commission's Annual Report Form

(This report is due April 30th of each year for the prior calendar year's gross Colorado Regulated Operating Revenues. Entities may not request an extension of time to file this report.)

Telecommunications carriers are required to collect and remit Telecommunications Relay Service surcharge - DOC

Colorado Low-Income Telephone Assistance Program (LITAP) Quarterly Payment Calculation

All Eligible Telecommunication Carriers certified to do business in the state of Colorado must file a report, as must providers of basic local exchange telecommunications services with approved LITAP tariffs on file with the Commission.

Colorado High Cost Support Mechanism (HCSM) Worksheet
  • All Telecommunications carriers providing intrastate telecommunications services within the state of Colorado must file a Colorado High Cost Fund Worksheet or De Minimis Declaration Form. 

  • Contributors that provide intrastate telecommunications, but whose contribution would be de minimis, are not required to contribute to the CHCSM. A contributor is considered de minimis if its contribution for the calendar year is less than $10,000. However, all Incumbent Local Exchange Carriers (ILECs) and Eligible Telecommunications Carriers (ETCs) must file the Worksheet and contribute to the CHCSM.
