Detailed description of prefix designations

A-
Application
Used when a company applies to: receive a Certificate of Public Convenience and Necessity ("CPCN") (fixed utilities and transportation utilities); alter their CPCN; receive approval of a refund plan; receive approval of a franchise agreement; issue stock (energy companies only); petition the Commission to arbitrate an interconnection agreement; abandon a CPCN; and request other remedies from the Commission that may not fall under a specific rule.
B-
Petition for Arbitration
Used when a petition is filed requesting that the Commission arbitrate specific terms of an interconnection agreement when the parties are unable to resolve the issues through negotiation.
C-
Commission Complaint
Used when the Commission determines that a utility or potential utility is violating the Statute or a Commission Rule.
D-
Request for Declaratory Ruling
Used when the Commission receives a petition from a utility or potential utility requesting that the Commission make a ruling whether the subject matter in the petition is jurisdictional to the Commission or not.
E-
Request - Rural Technology Enterprise Zone
Used when a request is received for a determination that investments in infrastructure qualify to receive applicable Rural Technology Enterprise Zone income tax credit.
F-
Formal Complaint
Used when a customer wishes to file a formal complaint against a utility rather than opting for mediation, or if mediation has failed. Additionally, one utility may file a formal complaint against another utility. These could involve billing disputes, quality of service, lack of service, etc.

Civil Penalty
Used when the Commission issues a Civil Penalty.
H-
Siting Disputes
Used when the Commission receives an appeal for dispute resolution where a public utility or power authority is denied a permit or application that relates to the location, construction, or improvement of major electrical or natural gas facilities.
I-
Investigation
Used when the Commission determines that something that a utility is doing or a rate that has already been approved by the Commission needs a closer look. In the case of a regulated Rural Electric Association, the Commission must allow proposed rate changes to become effective and do an investigation later rather than suspending and setting for hearing prior to the rate becoming effective as the Commission is allowed to do in the case of any other type of utility.

Discontinuance of Telecom Service Where No Customers Are Affected
Used when the Commission receives Notification of discontinuance or curtailment of a service or abandonment of a Certificate of Public Convenience and Necessity where the utility has no customers that will be affected by such discontinuance, curtailment or abandonment.
L-
Less than Statutory Notice Tariff Change
Used when a utility wishes to make a tariff change on less than the normal 30 day statutory notice (or in the case of a Competitive Local Exchange Carrier 14 days notice). The normal vehicle used to make a tariff filing on the prescribed notice period is an Advice Letter. However, if the company wishes to implement the tariff change sooner than the prescribed notice period, they must file an application to do so, with good cause shown.

Used when the pleading does not fall in any other category.

Used for Notices of Probable Violation for Gas Pipeline Safety.

Submittal of Gas Purchase Plans
Used when a gas utility files its annual Gas Purchase Plan for Commission review.

Inquiries
Used when the Commission wishes to establish a docket as a repository for responses to a specific inquiry.
R-
Rulemaking Case
Used when the Commission wishes to promulgate new rules or revise rules.
S-
Investigation & Suspension
Used when the Commission determines that a proposed tariff filed via an advice letter is unjust, unreasonable, or deficient in some way and wishes to hold a hearing on the matter.
T-
Telecommunications Interconnection Agreements
Used when two telecommunications companies file an interconnection agreement.
Note: for this type of docket there is no suffix
U-
Declaration of Intent to Provide Local Telecom Services in Rural Telecom Service Areas
Used when a competitive local telecommunications service provider intends to provide service in an area served by a Rural Incumbent Telecommunications Service Provider.
V-
Petition for Variance From or Waiver Of a Rule
Used when a utility files for a variance or waiver of a substantive rule.
X-
Exemptions
Used when a master meter operator files for exemption from rate regulation for provision of gas or electric service. A master meter operator operation is usually an apartment complex or mobile home park where the gas or electricity is not billed directly to the resident but billed in bulk to the complex or park. The direct cost of the gas or electricity must be passed on to the resident without profit to the master meter operator.
Z-

Number Utilization Requirement Waiver
Used when a telecommunications provider requests a waiver regarding number utilization requirements.
