Primary Source Verification
The
following are the basic items that we verify before we license
a podiatrist to practice in Colorado:
- Date of
birth (applicant must be over 21 years of age) is verified
by a copy of the birth certificate or passport.
- Graduation
from a Board approved podiatry school is verified by a certified
verification from the school.
- Internship/residency
(approved by Council on Podiatric Medical Education ) - 1
year completed program (minimum) is verified by a certified document
from that institution, if the licensee was originally licensed
after March 23, 2001.
- National
Board of Podiatric Medicine Examination Part 1 & 2 scores
and the PM LEXIS Examination Part 3 scores are sent directly
to this office from the examining agency.
- All states
in which an applicant may have an active, lapsed (or expired), educational
or temporary license or permit must send verification directly
to the Colorado Board (for purposes of licensure status, the terms "lapsed" and "expired" have the same meaning).
- Podiatrists
must provide proof of malpractice insurance as required
by Colorado law or claim a specific exemption, as permitted by
Board rule.
- The National
Practitioner Data Bank/Healthcare Integrity & Protection Data
Bank are queried for any actions.
The Board may
request more information before granting a license.
