Permits/Inspections

Permit Form - To install plumbing as a homeowner, plumbing contractor, gas piping installation, shower pan installation, or mobile home-modular setup. Fee information is included.
Map - State inspectors' territories by county in Colorado.
Variance Request Form - To request from the Board a variance to the currently adopted code on a specific plumbing installation. For additional information on variances, please see section 12-58-104.5 of the Plumbing Statutes.

Affidavit of Eligibility - This form must be completed by all applicants for licensure in order to verify legal presence in the United States.
Americans with Disabilities Act (ADA) Exam Accommodation - To request special accommodations.
Change of Address and/or Name Form - To submit an address or name change for your plumbing license.
Complaint Form - For additional information on the complaint process see section 12-58-110 of the Plumbing Statutes.
Felony Conviction Form - If you have ever been convicted of a felony, pled guilty or nolo contendere to a felony, or accepted a deferred judgment or deferred prosecution to a felony charge, you must complete this form.
Reapplication for Original License - If you have been approved for the exam and have not taken the exam in more than one (1) year, you must complete this reapplication.
SSN Affidavit Form - Affidavit for foreign candidates without a Social Security Number or non-immigrants in the United States on student visas.
