Pharmacy Information
All pharmacies with few exceptions must submit their controlled substance reporting transactions into the database.
Exemptions/Waivers
Hospital Pharmacies are exempt from the reporting requirement as long as they do not dispense more than a 24-hour supply of a controlled substance to an outpatient. Inpatient dispensing transactions do not need to be reported. If your pharmacy is exempt, please complete and return the following form to the Board:
Pharmacies that are not computerized may request a waiver. The Colorado State Board of Pharmacy will consider such requests in the ordinary course of business and approve or disapprove the request. If you wish to apply for a waiver, please complete and return the following form to the Board:
Pharmacies without DEA Registrations
If the pharmacy does not have a DEA registration, please complete and return the following form to the Board. Your pharmacy will be required to submit a “zero” finding into the program.
