In-State Manufacturers of Prescription Drugs (MFR)
A manufacturer with a facility located in this state must be separately registered to manufacture any prescription drug. In addition to initial registration and renewal, a manufacturer must submit an application and fee to apply for any change of ownership. An application must also be submitted for any change of name and/or location.
Each facility must be separately and appropriate registered.
NOTE: A properly registered manufacturer does not need registration as a wholesaler as long as it only distributes to the authorized distributor of record. If distribution is to any other person or entity, the manufacturer must also be registered as a wholesaler.

* NOTE: You must use the same application for new registrations and changes to existing registrations (i.e. transfer of ownership, business location or name change, or change of designated representative / consultant pharmacist / pharmacist manager).
