Request a Duplicate License

License Renewal. All Pharmacy business licenses expire on October 31 of even-numbered years. Renewal forms are mailed to the last known address on file approximately six (6) weeks prior to the expiration date.


Definitions:
  • Renew. You currently hold a Colorado license and are in a renewal period.
  • Reinstate. Your Colorado license has lapsed or expired and you wish to reinstate the license.
    (For purposes of licensure status, the terms "lapsed" and "expired" have the same meaning.)
