  Wall Certificates

License Renewal Information. All Occupational Therapist registration will expire on January 31 every two years in odd numbered years. Renewal forms are mailed to the last known address on file approximately six (6) weeks prior to the expiration date.


Definitions:
  • Original License. You are applying for an original Colorado license.
  • Renew. You currently hold a Colorado license and are in a renewal period.
  • Reinstate. Your Colorado license has lapsed or expired and you wish to reinstate the license.
    (For purposes of licensure status, the terms "lapsed" and "expired" have the same meaning.)
