License Renewal Information. All provisional licenses currently expire on August 31in odd-numbered years. Renewal forms are mailed to the last known address on file approximately six (6) weeks prior to the expiration date. NOTE: Provisional licenses are subject to termination upon a change in the licensee's employment or supervision. It is the licensee's responsibility to notify the Board of any change in supervision or employment within thirty (30) days.
