Mandatory Disclosure
Effective July 1, 2011, the passage of Senate Bill 11-187 included substantive changes to the content of mandatory disclosure forms. The new requirements include the notification of client rights both verbally and in writing, and listing the licensure/certification/registration requirements for all the mental health professions. To assist new applicants and existing licensees in formulating a disclosure statement that incorporates these requirements, provided is a sample mandatory disclosure form and a checklist:
