To file a complaint against an insurance company, click here to electronically fill out the form, or you can send a letter to the Division of Insurance at 1560 Broadway, Suite 850, Denver, CO 80202 (fax: 303-894-7455). You'll need to provide the company's name, your policy or ID number and a brief summary of your complaint.
Please be sure to send copies of any letters that are connected with your complaint, that  you've sent to, or received from, the company or other parties involved.

Complaints Against Insurers -- Report to Legislature, Oct. 1, 2009
The Annual Report on Consumer Complaints Against Insurers for
2008-2009 was provided to the Colorado State Legislature on Oct. 1, 2009, as required by statute. This year’s report provides comparable data for the last five years and identifies the top reasons consumers submit complaints to the Division regarding auto, health, life, homeowners, annuity and title insurance.
