License Renewal Information. All Funeral Home and Crematory registration will expire on November 30 every year. Renewal forms are mailed to the last known address on file approximately six (6) weeks prior to the expiration date.


Definitions:
  • Original License. You are applying for an original Colorado license.
  • Renew. You currently hold a Colorado license and are in a renewal period.
  • Reinstate. Your Colorado license has lapsed or expired and you wish to reinstate the license.
    (For purposes of licensure status, the terms "lapsed" and "expired" have the same meaning.)
