How do I request written verification of a license?
To request a license verification—including exam scores, if passed in Colorado—by mail, send your request to:

You may also fax your request to: (303) 894-7693.
With your request, you should include the licensee name and license number (if you have it) and the address where the verification is to be mailed. If you have a verification form that needs to be completed, include that with your request. There is no fee to process these requests.

If I am not licensed, how do I have my exam scores from Colorado verified to another state or professional organization for purposes of licensure or membership?
Many states and organizations provide a form to send to Colorado (CPA Exam Services) as part of their application packet. Some states require that you use only their form. If they do not, you can write CPAES, specify the information you need verified and provide the agency name and address where you want the verification sent. They will send a form letter. Please send your request in writing to:

Does Colorado charge a fee to verify only my exam grades to another state or professional organization?
Yes, CPAES charges a $25 fee made payable to CPA Examination Services.

You may make an online query of disciplinary documents via Registrations Online Documents (ROD) or you may send a written request to:
