24 Hour Incident Report

The following is the document required when the Licensee reports to the Board a twenty-four hour incident. These incidents are defined in Section 23 of the Colorado Passenger Tramway Safety Board's Rules and Regulations. The forms can also be printed for completion by hand or typewriter.

To complete the Incident Form, you will need the Log Code sheet for information regarding cause and failure codes.

Component Report

The following is the document required when the Licensee reports to the Board component failures. These incidents are required by Section 23 of the Colorado Passenger Tramway Safety Board's Rules and Regulations. The file is in a Microsoft Word version. The forms is in a protected format. The protected format will allow you to complete the form using a computer while not changing the form. The forms can also be printed for completion by hand or typewriter.

Items to be Completed Prior to Opening

During the licensing annual inspection, all of the following items may or may not be in place for the Inspector to observe. It is understood that with early season snow conditions, snow may be nonexistent and that you remove many of these items because of vandalism, construction etc., when the lift is not being operated for public operation.

It is the responsibility of the operator to have all of these items in place prior to operation of the lift for public use.
