Request a Verification of Your Colorado License
Colorado does not charge a fee for issuing a license verification. Send your request to:

With your
request, you should include the licensee name and license number
(if you have it) and the address where the verification is to
be mailed. If you have a verification form that needs to be
completed, include that with your request.
Please allow a minimum of two weeks for your request to be processed.
For Physicians and Physician Assistant licenses only:
Effective May 1, 2010, the Colorado Medical Board has outsourced license verifications for physicians and physician assistants to VeriDoc, a national physician licensure verification service. A verification of your Colorado medical license can be sent from VeriDoc to any medical or osteopathic licensing board in the U.S. This new service will expedite the processing of your verifications by electronically submitting them to other state boards in a matter of days. Please visit VeriDoc’s website at http://veridoc.org for an overview of their services, fees and additional information.
For RN and LPN Nursing licenses only:
All verifications are processed via Nursys™, the online database of nursing license information. If you are a Colorado nurse seeking license verification, you will no longer submit verification request forms directly to the Board. Instead, you will complete the entire verification process online by visiting the Nursys™ website.
For CNA Nursing licenses only:
Effective October 1, 2003, the Colorado Board of Nursing will no longer honor written or telephone requests to verify routine licensure status of RNs, LPNs, or CNAs. All of the necessary information may be obtained via the Internet using ALISON (Automated Licensure Information System Online). This system offers data available to the public for all professions and occupations licensed by the Division.
