Licensee Database List and Discipline List Requests:

You now have three options to obtain public information from our Licensee Database:

  1. Licensee Database List Download: This list is available immediately at any time by visiting our Database Request website and it is free of charge! Simply insert your search criteria regarding license type and status and the system will retrieve a text file that you may download and review.
  • The Database List Download includes: Licensee Names, Addresses, Professions, License Numbers, Original Issue Dates, Effective Dates, Expiration Dates, License Statuses and any available Authority, Specialty, Degree or Title information that may be available.
  • To access this list please click the following link: www.doradls.state.co.us/lic_database_req.php
  1. Licensee Database List Request: This list is available via the Licensee Database List Request form which must be mailed to the Division and requires a fee of $13 per license type requested.
  • The Database List Request includes: All information included on the Download listed above plus the License Method and Licensee Phone Numbers (if available). DISCLAIMER: This information is provided to you in accordance with statutory guidelines, which allow for the release of demographic information including name, address and - when available - phone number. However, please note that even though a phone number may be listed, it is your responsibility to comply with the provisions of the Colorado No Call Law and eliminate from your call list any phone numbers signed up for this service. You may view the no call website at www.coloradonocall.com.
  • To request this information please print, complete and mail to the Division the Database Request Form along with the appropriate fee in check or money order form made out to the State of Colorado. To access the Database Request Form, click here. You may use the "fill in" capability to type in your information to print and mail to the Division or you may simply print the form and fill in the document by hand.
  1. Licensee Discipline List Request: This list is available via Licensee Discipline List Request form which must be mailed to the Division and requires a fee of $50 per Board/Program requested.
  • The Discipline List Request includes: Complaint Number, Profession, License Number, Licensee Name, Discipline Action, Discipline Start Date and Discipline End Date.
  • To request this information please print, complete and mail to the Division the Database Request Form along with the appropriate fee in check or money order form made out to the State of Colorado. To access the Database Request Form, click here. You may use the "fill in" capability to type in your information to print and mail to the Division or you may simply print the form and fill in the document by hand.
