Department of Revenue Form DR 525 "ANNUAL REPORT OF PUBLIC UTILITY INTRASTATE
GROSS OPERATING REVENUE" - PDF must also be submitted to the Commission by April 30, 2012.
Entities may not request an extension of time to file Form DR 0525.
This DR 525 report MUST also be filed with the Colorado Department of Revenue on or before
May 1, 2012.

Utilities are strongly encouraged to have one of their designated filers submit an executable electronic copy (i.e., MS Excel workbook) in the appropriate proceeding
(Docket No. 12M-128R) via the Commission’s E-Filings system at:
https://www.dora.state.co.us/pls/efi/EFI.homepage.
Once in the E-Filings system, select the document type “Annual Report” and in the title field insert the utility’s name, along with “2010 Annual Report.” E-filing is a fast, efficient, and less costly way to submit filings to the Public Utilities Commission.
In the alternative, utilities may mail the signed original, one copy, and an executable electronic file on CD. Mail the hard copies and CD to the following address: Colorado Public Utilities Commission, 1560 Broadway, Suite 250, Denver, Colorado 80202-5143.

On or before April 30, 2012, each railroad, railroad corporation, and railroad carrier holding a Certificate of Public Convenience and Necessity to operate by rail in the State of Colorado during calendar year 2011, shall file an annual report in the form prescribed in this Order, and any additional reports as required by Appendix A to this Order.
Each filing entity shall also file a copy of the Colorado Department of Revenue Form DR 0525 no later than April 30, 2012. Entities may not request an extension of time to file Form DR 0525.
