License Renewal Information. All licenses expire on January 31 of even-numbered years. Renewal forms are mailed to the last known address on file approximately six (6) weeks prior to the expiration date.

If so, you can have these applications faxed directly to you.
Simply call (303) 894-7800, select option #6, then option #5,
and follow the instructions for the FaxBack program.


Definitions:
  • Original License by Examination. You are applying to take an examination in order to qualify for licensure in Colorado (CNA applicants must apply to Pearson Vue).
  • Original License by Endorsement. You hold an active license in another state and wish to apply for licensure in Colorado.
  • Renew. You currently hold a Colorado license and are in a renewal period.
  • Reinstate. Your Colorado license has lapsed or expired and you wish to reinstate the license.
    (For purposes of licensure status, the terms "lapsed" and "expired" have the same meaning.)
