The lists also do not include applicants denied initial licensure or reinstatement either before or after a hearing. Please note that licensees on this list may have since met Board requirements (for example, completed probation or a period of suspension). Obtaining Copies of Disciplinary Documents

Documents related to disciplinary actions are public information. If you wish to obtain a copy of a disciplinary document, you can view and print the document(s) created before February 2005 via Registrations Online Documents.

Documents created after February 2005 may be obtained via the Automated Licensure Information System Online (ALISON). Please note that it may take 6-8 weeks from the effective date of the disciplinary action for the documents to be scanned and posted on this site.
