The Colorado Division of Insurance (Division) is conducting a survey to obtain the most current information on PIP claims available in the state. All companies authorized to write automobile insurance in the State of Colorado must complete this survey request. This survey is currently open and is due no later than April 30, 2012. |

The Colorado Division of Insurance (Division) is conducting a survey to obtain the most current information on Medicare supplement plans available in the state. This information will be used in a publication that will be available to Colorado consumers at the Division of Insurance or via the Division's website. All companies authorized to write Medicare supplement insurance in the State of Colorado must complete this survey request. This survey is currently open and is due no later than April 30, 2012. |

Health Cost Survey FAQ |
The Colorado Health Cost Report is required for all companies licensed to write any health premium in Colorado pursuant to § 10-16-111(4)(a), C.R. S., and Colorado Regulation 4-2-31. The excel spreadsheet to the left should be downloaded, fully completed and returned to the Division via the dedicated web portal link along with a copy of the companies Supplemental Exhibit. No submissions will be accepted before May 1, 2012. If the web portal is down submissions will be accepted by email to ratesandforms@dora.state.co.us. This survey is due June 1, 2012. |
2011 Colorado Average Reimbursement Rate Survey |
Colorado law requires all health carriers who have written premium in 2010 to respond with tthe average reimbursement rates for the "twenty-five most common inpatient proceudres", based on the "most commonly-reported diagnostic-related groups". This survey is being made in accordance with HB 08-1393 and §10-16-134, C.R.Sl. with data reporting requirements oulined in regulation 4-2-31. This survey is due March 1, 2012. |

Then please update your company "Market Conduct Contact" information on file with the NAIC. This address may be updated by including it with the submission of the electronic version of the Jurat page with any annual or quarterly financial filings to the NAIC. The Market Conduct Contact is the Division's primary point of contact. However, you may provide additional assistance in updating more specific company addresses using our On-Line Address Maintenance system. This system allows insurance companies licensed in the State of Colorado to update their addresses and contact information used by the Division of Insurance.
