• Acknowledgment of Responsibility Form - For a master electrician who is the responsible individual for an electrical company.
  
• Affidavit of Eligibility - This form must be completed by all applicants for licensure in order to verify legal presence in the United States.
• Affidavit of Experience Form - For an applicant to document experience and work history (includes list of work experience definitions).
• Change of Address and/or Name Form - To submit an address or name change for your personal electrical license, or to submit an address change for your contractor's registration.
  • Electrical contractors MUST complete the Electrical Contractor Registration Change of Name Form.
    
• City/County Inspector Registration Form - To register city/county inspectors with the state board.
• Complaint Form - To file a formal complaint against an electrical company or an individual electrician.
• Felony Conviction Form - If you have ever been convicted of a felony, pled guilty or nolo contendere to a felony, or accepted a deferred judgment or deferred prosecution to a felony charge, you must complete this form.
• License Verification Request Form - For an applicant to request license verification from another state board.
• Natural Gas as an Alternate Fuel Source - To be submitted to the assigned electrical inspector when proposing to use natural gas as an alternate fuel for generators (NEC 700.12B3 Exc., 701.12B3 Exc).
• Reapplication for Original License - If you have been approved for the exam and have not taken the exam in more than one (1) year, you must complete this reapplication.
• Residential Inspector Application Form - To apply for appointment as a residential inspector in the State of Colorado.
• SSN Affidavit Form - Affidavit for foreign candidates without a Social Security Number or non-immigrants in the United States on student visas.
• Variance Request - To request a variance to the currently adopted code on a specific electrical installation.
• Woodworking Facilities – For Group E, Educational occupancies ONLY. Outline of procedures to declassify.
